# 【全口無牙重建案例】數位化微創全口植牙固定修復

## **前言**

數位化微創全口植牙固定修復能顯著改善全口無牙患者的咀嚼功能與顏面美觀，當臨床條件允許時，已逐漸成為部分無牙患者的有效治療選項。然而，無牙患者通常年齡較長，整體身體機能相對年輕族群下降，加上全口重建本身具有手術風險高、修復難度大等挑戰。作為患者口腔功能重建的最終整合治療階段，往往也伴隨患者對療程的不安與焦慮。

全流程數位化治療可涵蓋術前規劃、患者溝通、植體手術至修復階段的每個環節，協助醫師提高精準度、減少侵入性並提升患者信心。本文將分享一例微創式無牙顎即刻種植與同步製作永久性整體式固定橋架的臨床案例，期盼與各位先進共同探討數位化治療流程的臨床價值與應用意義。

 

## **患者基本情况**

患者為一位 75 歲男性，身體狀況良好。自 30 年前開始，全口牙齒陸續鬆動拔除，其間曾多次接受活動式假牙修復。五年前因原活動義齒配戴鬆動，再次製作全口義齒，近期再次出現義齒固位不良，並於進食時感覺下顎牙齦酸脹不適，因而前來本院諮詢進一步治療方案。

患者面部外觀對稱，配戴活動假牙時下三分之一臉型較為飽滿，雙側顳顎關節按壓無痛，張口度與開口型態正常，無彈響聲。口內檢查顯示目前活動義齒在靜態咬合時尚屬穩定，但咀嚼運動時固位力不足，進食與說話時有脫位現象，上顎義齒唇側出現破裂。上下顎齒槽骨皆呈明顯吸收狀態，牙槽脊呈低平型態。下顎牙槽骨吸收程度較重，牙槽脊高度已近前庭溝位置，雙側牙槽脊頂部（約4-5號牙區）有壓痛與酸脹感。

上下顎角化牙齦充足且組織致密，軟組織無紅腫或潰瘍現象。CBCT 顯示：上顎骨關係為中性咬合，上顎左側後牙區骨高度略顯不足，其餘區域骨量充足，骨質為 D3（即 III 類）。下顎方面，頦孔前區骨量充足，頦孔後區骨密度顯著不足，高度約僅剩 1–3 mm，頦孔位置偏向牙槽脊頂部的頰側。

將舊義齒組織面重新重衬並置入放射性標記點，患者配戴舊義齒於咬合狀態下拍攝 CBCT，藉此取得顎骨、咬合與臉型關係的影像資料。同時拍攝舊義齒作為放射義齒，作為後續數位化植牙規劃之依據。

下顎各區骨量與骨質分析：

下顎頦孔前區骨量充足，頦孔後區骨密度顯著不足，垂直高度約為 1–3 mm。

頦孔位置偏向牙槽脊頂部的頰側，骨質屬於 D2–D3 類（II–III 類骨質）。

上顎左側後牙區骨高度略顯不足，其餘區域骨量充足，整體骨質為 D3–D4 類（III–IV 類骨質）。

## **診斷：全口無牙症（Complete Edentulism）**

### 治療計畫終印模

綜合目前臨床與影像資料評估，患者為全口無牙狀態，治療選項包括傳統全口義齒、覆蓋式義齒修復（Overdenture）以及全口種植固定式修復。考量下顎可用骨量有限，是否適合進行全口種植固定式修復，仍需進一步完成修復設計評估後再做決定。

在治療創傷程度方面：  
全口義齒 ＜ 覆蓋式義齒 ＜ 全口種植固定修復

在固位力與咀嚼功能恢復方面：  
全口義齒 ＜ 覆蓋式義齒 ＜ 全口種植固定修復

與患者充分溝通各治療方案之優劣後，考量其下顎後牙區頦孔位於牙槽脊頂部，若選擇活動式修復（全口義齒或覆蓋式義齒），未來有可能因頦孔受壓導致不適，需經常進行調整與重衬。

最終，醫療團隊與患者傾向選擇全口固定式種植修復方案，並規劃使用 3Shape Implant Studio 進行數位化種植與修復設計。

全口種植修復方案設計

透過初診流程，我們已取得包含放射性標記義齒、CBCT 影像與咬合資訊的完整資料，並將其匯入 3Shape Dental System 進行橋架修復設計，同時使用 3Shape Implant Studio 進行植體位置規劃，據此制定最終治療計畫。

上顎預計植體位置為 12、14、16、22、24、26，其中 26 號部位進行簡單的上顎竇內提升術；下顎植體位置為 25、22、32、35，其中 25、35 號植體略向遠中傾斜，以增加 AP 距離、減少後方懸臂長度。

在方案確認後，我們進行手術導引板的設計與製作。本案例導板採雙側設計，保留原始咬合資訊以利手術時就位穩定。由於本案例中下顎導板可能因旋轉而造成就位誤差，並有潛在傷及頦神經的風險，因此手術前先進行導板預就位後拍攝 CBCT，並重建確認其位置精準。

如圖所示，導板就位精確，因此本病例可安全採用微創式植牙手術方案，以縮短手術時間並減少手術創傷。

## **治療過程：**

### 植牙手術

術後即刻口內照

手術導板咬合就位後，先於唇側進行局部麻醉並置入固位釘，再於腭側追加麻醉並置入第二支固位釘。補充唇頰側局部麻醉後，正式開始進行微創植牙手術。為有效縮短手術時間，本次手術過程未進行術中攝影記錄。

手術順利完成，所有植體皆獲得良好的初期穩定性，達成預期植入目標。

術後進行常規印模與咬合關係轉移，完成即刻修復作業。

### 永久修復階段說明

### 由於患者就診期間時間有限，暫時修復體配戴約八個月後，出現後牙牙冠脫落情形。為維持患者日常咀嚼功能，我們依據原暫時修復體之咬合資訊，製作第二階段的暫時修復體，以延續功能並穩定咬合關係，為後續永久修復做準備。

### 永久修復在暫時修復基礎上進行顎位關係轉移，分析發現患者咬合高度需進一步調整。因此我們以口掃進行取模，同步記錄調整後之咬合高度，並將暫時假牙、掃描柱與 CBCT 資訊整合，匯入 3Shape Dental System 進行虛擬面弓轉移與最終修復設計製作。

### 修復體戴入口內後可見，其與基台密合良好、咬合穩定，色澤與形態自然美觀，成功達成預期之臨床效果。
