## **一位牙醫師的數位牙科探索之路**

踏入口掃數位領域至今已有七、八年，我幾乎見證了口掃機的整個演進史。從早期的 CareStream 3500 / 3600，到後來的 Omnicam、TRIOS 3，再到近幾年市場上備受關注的 Medit i700 / i500 與評價不錯的 PrimeScan，每一台設備我都曾深入使用。

如果要我評價，我會說每台口掃機都有他的優缺點，這算是一個時代的演進產品，而譽辰齒研生技獨家總代理的 3Shape TRIOS 5 應該是已經把很多前面口掃機的缺點都已經做了更新，例如 CS3500 / 3600 它口掃的速度，我相信如果有用過，應該是都會知道，慢慢的一張一張照片去掃，它的速度是非常緩慢的，後來 T3 其實就已經把這個速度提升了很多，而現在 T5 的部分速度已經是非常快，再加上 T5 是無線的(前面幾台口掃機都是有線裝置)，無線就是非常的簡潔、方便，不會有一條線在那邊纏住你，因為是無線，所以不用移動推車。

T5 就算是有一點距離也可以輕鬆口掃取得患者口內影像。T5 的 Share 功能在同一網域可以做到跨診間、跨樓層的使用，至於 T5最強的優勢真的在於口掃的速度，無論是和 T3、Medit、Omnicam 相比在口掃速度上還是優於他們，3Shape 軟體的穩定度也相對穩定，以上是我現在最主要使用 T5 並對它評價很好最主要的原因。

若和 T3 相比，通常我們使用的 T3 都是有線機型，當然也有無線版，但是 T3 的無線機型在軟體使用上沒 T5 那麼順暢，以 T3 有線機型來說，它常常會有一條線，直接放在病人的胸前，或是從病人的頭後面繞過去，操作上有時候會覺得這條線卡卡的。所以無線的系統導入進來，而且它的操作上是完全不會有任何的延遲，我幾乎都依賴譽辰齒研生技獨家總代理的 T5 來完成數位取模的工作，患者也對無線操作的部分評價很好。

## **極致便利的操作與續航力**

T5 的無線操作是我認為最便利的地方，尤其是遠端遙控的部分，你只需要觸控 T5 的按鈕，就可以控制你在螢幕上的影像作業，不用去觸控電腦螢幕或是動用滑鼠，你就可以去點選你想要的選項，完全不需要動用滑鼠，不僅衛生還提升了操作的便利性。T5 的便利性除了無線之外還有電池，無線大家最大的疑慮就是電池的續航力，T5 一顆電池大概可以用一個多小時左右，再加上他有三顆電池，三顆電池它不會讓你沒電使用的機會。

## **精準度的再次突破，攻克臨床難點**

TRIOS 軟體的掃描精確度跟掃描速度在 T5 身上有帶來很明顯的突破嗎？因為我也使用過 T3，我覺得 T5 的精準度的確實比 T3 來的強，還要更快，精準度也是有明顯的感受到，我現在無論是活動假牙、固定假牙，尤其是植牙，我幾乎全都使用T5來口掃。

T5 相較於前幾代，我覺得它最厲害的地方，是在無牙脊口掃，確實比 T3 更清楚，而且很容易可以抓到，無牙脊掃描，可以很迅速的回到影像沒有疊合好的部分，因為可動黏膜很容易造成掃描失敗，T5 的動態掃描模式可以準確的提高影像掃描準確度。尤其在無牙脊口掃印模上，我也用由譽辰齒研生技獨家總代理的 3Shape TRIOS 5 來取代，精準度方面它在這兩側的遠心游離端，像 T3 的部分我覺得會有一點變形，對這部分評價不是很好，但是 T5 的變形已經縮到很小，所以包括後牙植牙贋復，都可以直接以口掃來取代，精準度更勝印模。

用 3Shape TRIOS 5 數位口掃無牙脊部分幾乎不太會跑掉，比我們用 Alginate 還要來得更準確，比如掃舌側，我們可以決定口掃的深度，但是你印 Alginate 的時候，你很難當場判斷要印到哪一個深度，都要拔起來再看，但是口掃是即時的，你可以直接判斷深度，尤其後側面的 retromolar pad 要掃多深，都是可以用口掃去控制，但是我覺得無牙脊的口掃是需要時間練習，沒有練習真的是掃不起來，像我會把患者的嘴巴一次就撐開，盡量一次完成所有的掃描，偶爾補一些地方的影像，無牙脊的口掃跟自然牙口掃會差蠻多的，確實是需要點時間練習。

## **從臨床應用到患者回饋：為何它值得信賴？**

其實你今天不管用哪一代的口掃機，很順利很快速的完成印模，他們都會覺得這是一個很神奇的事情，不管是 T3、 T4 、T5，其實要做到這件事情已經是很簡單了，得到的評價也都差不多，因為我們就是很快速的掃過去，立即抓到口內的影像，病人其實躺下大概 1-2 分鐘上下顎可以全部掃完，對於他們來說是一個很神奇的體驗，評價也都很正向，他們幾乎常常都會說：「哇，你們這邊很先進！」因為患者以前的印模體驗是非常不舒服的，我想這是病人帶來最主要的回饋，夠快、又準、舒適的體驗是最真實的評價。

我現在幾乎都是靠譽辰齒研生技獨家總代理的 TRIOS 5 來完成所有的贋復工作，包括單顆的 Single Crown、Bridge，到植牙贋復及活動假牙，甚至到雙重冠的活動假牙都是利用口掃來完成，除非這個患者他的情況不允許。

例如我曾經遇過一位患者的舌頭真的是很大，然後他會把他 ridge 都蓋住，那時候還是使用 T3，T5 到目前為止我還沒有遇到不能掃的情況，尤其在我們醫院常會遇到一些口腔癌的患者，口腔癌的患者張嘴困難度比較高，所以我基本上現在都是以口掃來取代傳統印模。

## **為什麼我會推薦 TRIOS 5？**

針對技術支援的評價，只能說 3Shape 口掃機的軟體穩定度有相當的水準。我每次有問題只要打給業務，他們就直接說約時間線上遠端就解決了我的問題，我覺得是蠻快的，處理也相當及時，很多問題線上就解決了，就算線上沒有解決，技術人員也會馬上來幫我處理，對這部分的評價很好。使用將近兩年的時間，目前來說 TRIOS 5 基本上沒遇到什麼問題。

我覺得是可以加速看診的流程，因為你從前端的數位印模是準確的，所以你整個假牙的製程可以減少很多的誤差，比如說補綴物裝置，因為你前面口掃的精確度夠準確，所以技師可準確的產出補綴物，你可以很快速的把假牙裝在患者身上，提昇患者的治療體驗及正面評價，也可以提高臨床的翻桌率。尤其在植牙贋復的時候，當你的口掃已經夠精準了，那你做植牙贋復出來的這個效果也會越好。
